A two-stage, multi-method educational training
will be used to accomplish the below stated objectives using the Continuum of Care model. County faculty who participate in this training will understand how to apply the Continuum of Care model to specific issues related to caregiving.
  1. Rate functional health status based on basic and instrumental activities of daily living.

  2. Identify how functional health status influences decisions related to housing.

  3. Distinguish between functional housing options.

  4. Understand changing and challenging family relationship issues.

  5. Identify communication strategies to develop family caregiving plans.

  6. Recognize and respond to such psycho-social issues as loneliness, depression and grief.

  7. Explore financial costs and legal considerations.

  8. Identify and evaluate the quality of local resources.

  9. Recognize various fraud schemes targeted to caregivers and older people.

  10. Be prepared to conduct educational programs on the above topics.

  11. Develop a strategy to network with other caregivers.

  12. Include topics addressed in this in-service in their 2006 Plan of Work.
